Instructions

Step 1: Start with this form below. This is the family registration to establish your login and password for the Members Page, provide the contact information for parents/guardians, and acknowledge the Waiver and Liability release.

Step 2: After you complete this form, you will be taken to the Members Page. On the Members Page you will add the players that are associated with this family. Make sure when you enter your players that you provide accurate information. This will determine the programs that are available for each player.

If you have more than one player, programs may qualify for a sibling discount. The sibling discount is calculated on the Members Page and is determined by what programs have been selected.

After you have added the family information and player information it will be saved so that when you return to the site it will keep this information. From the Members Page, you can edit/update the family and player information.
